To request the use of Meadowbrook facilities or to submit an announcement to the church please fill out the following information.

What means will you use to promote your event? All promotional materials (posters, flyers for mailboxes, banners, announcements) must be submitted to the office for approval. Bulletin announcement Expanded Website announcement Verbal announcement from pulpit PowerPoint announcement Posters (eight sites in church building to choose from, based on availability) Flyer for mailbox Banner space -- reserve Lobby table or space at the information counter (call church office for availability). Limit of two tables on a given Sunday for a maximum of three consecutive weeks.

Number of Rooms Needed (Approximate number of people attending - if over 50 a set up and take down team of 4 people from the ministry will be required to assist the building manager.):
